Key to MoD sighting report form

The following text is taken from the MoD standard UFO reporting form. It appears that there were a number of different versions of the "official" form in circulation in 1993, the variants are covered below. In most cases, military signals indicate the section to which the comments following apply, eg:

A. 31MAR93 01:10Z 1 MIN
B. TRIANGULAR, SIZE OF FOOTBALL FIELD, BRIGHT WHITE LIGHT AT EACH CORNER, WITH RED AND GREEN LIGHT ON EITHER SIDE.
C. HIDING UNDER GARDEN TABLE, REAR GARDEN, MILTON KEYNES.
D. VIEWED THROUGH VIDEO CAMERA, ZOOMED IN TO 220x
E. ....

A. Date, Time & Duration of Sighting

B. Description of Object (No of objects, size, shape, colour, brightness)

C. Exact Position of Observer Location, indoor/outdoor, stationary/moving

D. How Observed (Naked eye, binoculars, other optical device, still or movie)

E. Direction in which Object first seen (A landmark may be more useful than a badly estimated bearing)

F. Angle of sight (Estimated heights are unreliable)

G. Distance (By reference to a known landmark)
(Variant: G. . Angle of sight).

H. Movements (Changes in B F & G may be of more use than estimates of course and speed)

I. Met Conditions during observations (Moving clouds, haze, mist etc)
(Variant: no item "I").

J. Nearby Objects (Telephone lines, high voltage lines, reservoir, lake or dam, swamp or marsh, river, high buildings, tall chimneys, steeples, spires, TV or radio masts, airfields, generating plant, factories, pits, or other sites with floodlights or night lighting)
(Variant: J. Met Conditions during observations (Moving clouds, haze, mist etc)).

K. To whom reported (Police, military, press, etc)
(Variant: K. Nearby Objects (Telephone lines, high voltage lines, reservoir, lake or dam, swamp or marsh, river, high buildings, tall chimneys, steeples, spires, TV or radio masts, airfields, generating plant, factories, pits, or other sites with floodlights or night lighting)).

L. Name & Address of Informant
(Variant: L. To whom reported (Police, military, press, etc)).

M. Background of Informant that may be volunteered
(Variant: M. Name & Address of Informant).

N. Other Witnesses
(Variant: N. Background of Informant that may be volunteered).

O. Date, Time of Receipt
(Variant: O. Other Witnesses).

P. Any Unusual Meteorological Conditions
